Position Description:<\/b><\/p>\n

\nPositions in this function (with the exception of Healthcare Services Coordinator<\/b>) require education \/ certification \/ licensure other than an RN, such as BSW, MSW, Respiratory Therapist, Dietician, Nutritionist, Occupational Therapist, etc. Positions are responsible for providing clinical and medical management services, including case management, health assessments, interventions, and discharge planning. May identify, coordinate or provide appropriate levels of care under the direct supervision of an RN or MD. This function does not include Health Coach, Health Educator and Health Advocate roles that do not require an RN. Those roles are found in the Wellness Coach Management function.

\nPrimary Responsibilities:<\/b><\/p>\n

    \n
  • Providing exceptional customer service<\/li>\n
  • Multitasking, inbound \/ outbound phone calls to members and \/ or physicians<\/li>\n
  • Completing assessments and documentation within a CER system<\/li>\n
  • Provide benefit explanation<\/li>\n
  • Health care knowledge surrounding disease management, community resources, durable medical \/ consumable products<\/li>\n
  • You will be providing care coordination telephonically; completing assessments and documentation within a clinical system while multitasking other applications gathering information to support member’s needs<\/li>\n
  • Must have excellent typing, writing and grammar skills<\/li>\n
  • Primary responsibilities are telephonic case management and some administrative responsibilities for the clinical department related to member care<\/li>\n
  • Very fast paced environment. 10+ people, team player, goal focused, must be a self-motivated, take initiative and willing to learn in a fast environment<\/li>\n
  • Moderate work experience within own function<\/li>\n
  • Some work is completed without established procedures<\/li>\n
  • Basic tasks are completed without review by others<\/li>\n
  • Supervision \/ guidance is required for higher level tasks<\/li>\n
  • Role assists in coordinating health services and does not write care plans<\/li>\n<\/ul>\n

    Required Qualifications:<\/b><\/p>\n

      \n
    • High School Diploma or GED<\/li>\n
    • 2+ years of experience working in a case management or a telephonic healthcare environment<\/li>\n
    • Experience with Microsoft Outlook (send emails and set folders) and Microsoft Excel (update spreadsheets)<\/li>\n
    • Experience with medical terminology, benefits management, claim, and \/ or healthcare insurance<\/li>\n<\/ul>\n
