Full-Time Nurse, Manager Clinical Services Home Health<\/strong>
\nFarmington Hills, MI<\/p>\n

If you’re a registered nurse with at least two years of experience in home health we hope you’ll consider becoming our clinical services manager. It’s a position where you’ll develop leadership skills and forge relationships that can\u00a0open opportunities\u00a0to advancement. You’ll ensure our\u00a0home health\u00a0team delivers high quality of care\u00a0while providing ongoing case management for all of our patients. You’ll touch the lives of both our patients and care providers. As an educator and mentor you’ll find ways to address deficiencies in clinical standards, care and compliance, along with improvements to OASIS and medical records. We’re looking for someone who will provide constructive feedback to our associates and review agency outcomes. \nYour responsibilities: <\/strong>
\n* Supervise, monitor, and ensure competencies and performance of an assigned home health care team and assuring the delivery of quality home health services
\n* Provide ongoing case management of all patients assuring an accurate evaluation and treatment plan, maintaining our quality model, and utilizing services appropriately to achieve optimal outcomes
\n* Review electronically submitted documentation and provide feedback to clinicians regarding deficiencies or OASIS corrections to ensure the medical records reflect compliance with medical necessity, homebound status, visit utilization supported by individual patient assessment\/ documentation support and transition (discharge) planning
\n* Review and approve final Plan of Care (485) within Electronic Medical Records (EMR) system
\n* Review patient schedules to ensure appropriate coordination of care and clinical needs
\n* Work directly with the assigned home health care team with the ongoing implementation of approved work methods, patient care model, and procedures that reflect elements essential to rendering high quality care <\/p>\n

Job Requirements<\/h3>\n

Required skills and qualifications: <\/strong>
\n*\u00a0Valid RN license within practice state
\n* Two years of experience within a licensed and certified (Medicare) home health agency.
\n* Competent in Federal (Medicare) and Local standards including Conditions of Participation and Local Coverage Determinations
\n* Basic level computer skills including Outlook, Word and Excel
\n* Must enjoy working with the senior population<\/p>\n
